Early investigations suggest that the coast guard plane, which had been tirelessly assisting earthquake-stricken areas, misunderstood air traffic control's instruction to hold short of the runway.
Unfortunately, Tokyo Haneda's runway collision warning lights were not operational, and the air traffic controllers failed to notice the runway monitoring system indicating the coast guard plane's presence for 43 seconds before the accident.
